Nurse Practitioner

We are seeking a compassionate and dedicated Registered Nurse to join our team at St Vincent's Care Service, Haberfield. As a key member of our care team, you will be responsible for providing high-quality care and support to our residents, ensuring their physical, emotional, and social needs are met.
As a Registered Nurse, you will work collaboratively with our multidisciplinary team to deliver person-centred care, promoting the independence and dignity of our residents. You will be responsible for assessing, planning, implementing, and evaluating individualized care plans, ensuring the best possible outcomes for our residents.
Key Responsibilities- Provide high-quality nursing care to residents, including administering medications, wound care, and other treatments as required
- Assess and develop individualized care plans, working collaboratively with the care team to ensure resident needs are met
- Conduct regular reviews of care plans, making adjustments as necessary to ensure optimal resident outcomes
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Participate in quality improvement initiatives, contributing to the development and implementation of best practice care standards
- Nursing Registration with the Australian Health Practitioner Regulation Agency
- Clinical nursing practice experience in an aged care facility, with a minimum of 2 years' experience
- Flexibility and availability to work PM shifts, Thursday, Friday, and Saturday
- Leadership skills, with the ability to lead a team by example
- High level of customer service skills, with a focus on resident-centered care
- Strong attention to detail and excellent communication skills
- Ability to follow directives as per care plan
- Excellent record-keeping skills, with experience using Autumn Care (or similar) desirable
- Understanding of Aged Care and resident rights and responsibilities
