Job Description

HammondCare is a Christian charity dedicated to improving the quality of life for people in need. We provide innovative health and aged care services, focusing on relationship-based care that values each individual as a whole person.

The Pastoral Care Coordinator plays a vital role in delivering this care. Reporting to the Pastor al Care Manager, you will utilise your pastoral expertise to support the emotional and spiritual needs of clients, their families, and staff.

This permanent part-time role involves working Wednesday to Friday at our residential site in Melbourne. You will:

  • Provide pastoral care and support to residents, families, and staff;
  • Be a point of contact for internal and external support/referral services;
  • Act as a role model for staff and advocate for HammondCare's Mission, Motivation, and Mission in Action, and its Christian values;
  • Share pastoral expertise and coach staff to support spiritual needs;
  • Conduct assessments of resident/family needs and goals using the HOPE assessment tool;
  • Develop spiritual care plans, working with staff to ensure client/family spiritual needs are acknowledged;
  • Participate in case management reviews, contributing to the development and evaluation of care plans;
  • Lead regular carer support groups;
  • Network with religious leaders and coordinate relevant care, resources, and group activities;
  • Strategically develop tailored resources, ideas, and activities to enhance spiritual and emotional wellbeing;
  • Communicate and build trust by developing relationships with staff and volunteers.

You will also:

  • Train staff on palliation and wellbeing;
  • Officiate interdenominational church services;
  • Lead memorials, ANZAC, and special services;
  • Lead and train a team of pastoral care volunteers;
  • Work collaboratively within the Pastor al Care team.
