Make a Difference in the Lives of Older People

You will provide strategic oversight and mentorship to our care professionals, ensuring older individuals with complex needs receive safe, responsive, and person-centered support in their homes and communities.

  • Provide case management guidance, mentoring, and support to care professionals.
  • Review assessments, care plans, and ensure services align with client goals and preferences.
  • Champion person-centered and trauma-informed care that respects dignity, rights, and culture.
  • Partner with clients, families, and health professionals to improve care outcomes.
  • Monitor risks, manage transitions of care, and resolve clinical concerns.
  • Ensure compliance with aged care legislation, quality standards, and organizational policies.

Requirements

  • A current registration with AHPRA in Nursing, Allied Health, or a related clinical field.
  • Experience in complex case management, ideally in community, aged care, or complex care.
  • Strong knowledge of aged care legislation, standards, and clinical governance.
  • Excellent communication and mentoring skills to guide staff and support clients.
  • A valid driver's license, NDIS worker screening, or national police clearance, and reliable transportation.
