Job Summary
We are seeking a highly skilled and experienced Registered Nurse/Enrolled Nurse to join our team as a Care Coordinator. As a key member of our care team, you will be responsible for coordinating and delivering high-quality care services to our clients, working closely with our care staff and management team to ensure seamless delivery of care.
Key Responsibilities
- Client Care
- Develop and implement individualized care plans that meet the unique needs and goals of each client
- Coordinate and deliver care services, including medication management, wound care, and personal care
- Monitor and evaluate client progress, making adjustments to care plans as needed
- Team Collaboration
- Work closely with care staff to ensure effective communication and coordination of care services
- Provide guidance and support to care staff to ensure high-quality care delivery
- Compliance and Quality
- Ensure compliance with relevant laws, regulations, and industry standards
- Contribute to the development and implementation of quality improvement initiatives
Requirements
- Current registration with AHPRA
- Satisfactory Australian National Police check
- Current or willing to obtain a first aid and CPR certificate
- Professional Indemnity Insurance
- A smart mobile phone (iPhone/android)
- Driver's licence and reliable presentable car
- COVID-19 vaccinations
