Job Opportunity

We are seeking an experienced Care Manager Lead to join our team in Burnett Heads. As a key member of our service, you will play a pivotal role in providing exceptional support to individuals with physical and psycho-social disabilities.

About the Role

This is an exciting opportunity for a dynamic leader who is passionate about delivering person-centered care. You will be responsible for overseeing daily operations, coordinating individualised care plans, and fostering a collaborative environment that empowers both team members and our clients.

Key Responsibilities:
  • Oversee daily operations of the Home Service site to ensure effective management of resources.
  • Ensure team member awareness of each client's requirement and provide support aligned to their plans, goals, and needs.
  • Deliver and encourage accurate documentation of all activities to maintain an audit trail and progress notes.
Requirements:
  • Experience in the Disability sector, particularly in managing complex behaviours and mental health challenges.
  • Relevant qualifications, such as Certificate III in Disability and/or Certificate III in Individual Support, are highly regarded.
  • Proven interpersonal and communication skills to build strong relationships with team members and stakeholders.
  • Problem-solving skills to successfully identify problems, develop solutions, and implement them using a logical and systematic approach.
  • Availability to work flexible hours, including some weekends and overnights, as required.
  • Holding or being willing to obtain relevant certifications, such as a Working with Children Check, NDIS Worker Screening Check, First Aid, CPR, and National Criminal History Check.
