The successful candidate will work collaboratively with healthcare professionals to promote safe, effective, and cost-efficient drug therapy practices. This includes medication reconciliation, patient profiling, dispensing medications, and providing drug information to support optimal patient care.
This role is ideal for a motivated individual who enjoys working in a dynamic environment and is committed to delivering exceptional patient outcomes.
Key Responsibilities- Provide clinical pharmacy services to patients throughout the South West region, including medication reconciliation and patient profiling
- Work collaboratively with healthcare teams to optimize patient care and treatment plans
- Contribute to the development and implementation of clinical pharmacy programs and initiatives
To be successful in this role, you will need:
- A degree in pharmacy or equivalent qualification
- Current registration as a pharmacist with the Pharmacy Board of Australia
- Excellent communication and interpersonal skills
- A strong understanding of clinical pharmacy principles and practice
