About the Role:
We are seeking a highly motivated and experienced Social Worker to join our Ambulatory Aged Care Programs team. As a Social Worker, you will be responsible for providing high-quality social work interventions to a diverse consumer population.
Key Responsibilities:
- Provide psychosocial assessments, counseling, support, case management, crisis intervention, and advocacy to patients, their families, and significant others.
- Develop and implement individualized care plans to meet the unique needs of each consumer.
- Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
- Conduct regular reviews and updates of consumer care plans to ensure ongoing relevance and effectiveness.
- Provide education and support to consumers and their families on social work-related topics.
Requirements:
- Approved tertiary qualification in Social Work.
- Membership and/or eligibility for membership of the AASW is maintained and working within scope of practice.
- Well-developed time management and critical thinking skills, the ability to prioritize tasks and work under minimal supervision.
- Excellent written and verbal communication and interpersonal skills and the ability to work effectively within a team environment.
- Sound knowledge of computer-based software applications including Word/Excel/database management systems.
