Chronic Health Nurse
The Chronic Health Nurse is responsible for navigating care of the patient and services that facilitate a person with a chronic disease to manage their health in a way that will result in optimal health outcomes.
A creative, flexible and adaptable person who is able to think outside the box and be good problem solver whilst managing a vulnerable and complex cohort of patients.
Providing care in a range of modes and settings including home visits, MDT clinics, virtual care and group classes.
Collaboration with many external stakeholders including but not limited to Primary Health Network, NDIS, My Age Care, other Local Health Districts and Non-Government Organisations.
About You- Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
- Relevant post-registration qualifications and at least three years' experience working in the clinical area of their specified post-graduate qualification.
- Current unrestricted NSW Driver's License.
- Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
- Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.

In this role you will be based at a busy inner-city hospital in a beautiful location surrounded by great cafes, galleries and theatres, parks, nightlife, and nearby glorious beaches.
A place of expert clinical care for at-risk people closely partnered with universities & affiliated research institutes, many of which are located on-campus.
Centres of excellence include heart & lung transplantation, bone marrow transplantation, cardiology, cancer care, AIDS/HIV, respiratory medicine, mental health, homeless health, drug & alcohol services, and correctional health.
