Job Overview

We are seeking an experienced and compassionate Care Experience Coordinator to join our team. The successful candidate will be responsible for delivering exceptional daily living experiences to residents in a care setting.

Key Responsibilities
  • Coordinate daily activities and operations within the household, ensuring seamless integration of services and support.
  • Collaborate with residents, families, and other roles to develop tailored care plans that cater to individual needs and preferences.
  • Facilitate social interactions and community engagement, promoting a sense of belonging and connection among residents.
  • Provide emotional support and reassurance to residents, responding to their changing needs and concerns.
  • Work closely with healthcare professionals to ensure comprehensive care and support services are delivered effectively.
  • Maintain accurate records and reports, monitoring resident outcomes and program effectiveness.
Requirements
  • A minimum of 2 years' experience in aged care or a related field, with a focus on care coordination and delivery.
  • Diploma or degree in a relevant field, such as nursing, social work, or healthcare management.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and staff.
  • Ability to work collaboratively as part of a multidisciplinary team, prioritizing resident-centered care and well-being.
  • Strong problem-solving and adaptability skills, with a commitment to continuous learning and professional development.
