Aged Care GP Program Specialist

We are seeking a skilled and experienced Aged Care GP Program Specialist to join our team at Trust Medical Recruitment. As a key member of our organization, you will play a vital role in providing high-quality healthcare services to residents in aged care facilities.
Key Responsibilities:- Provide medical services to residents in aged care facilities, utilizing a combination of face-to-face consultations and virtual care.
- Conduct regular routine consultations to monitor residents' health and prevent hospitalizations.
- Collaborate with a team of healthcare professionals, including doctors, nurses, geriatricians, and allied health professionals, to ensure residents receive comprehensive care.
- Develop and implement effective care plans to improve health outcomes and quality of life for residents.
- Work closely with facility staff and healthcare professionals to ensure seamless coordination of care and clinical handover.
- Vocational or General Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Full Australian work rights.
- Experience in Emergency Department (ED) and/or General Practice.
- Ability to work independently and as part of a team.
