Job Summary

We are seeking a skilled and compassionate Case Manager to join our team at Life Without Barriers. As a Case Manager, you will play a pivotal role in providing holistic support to young people in need.

About the Role

In this role, you will provide quality case management support for Children & Young people residing within our therapeutic care homes. You will work closely with the Children, Young People and their support network to develop and implement therapeutic case plans that meet their individual needs and goals.

Key Responsibilities

  • Provide case management support to Children & Young people in need
  • Develop and implement therapeutic case plans that meet individual needs and goals
  • Work closely with Children, Young People and their support network
  • Collaborate with internal and external stakeholders to ensure comprehensive support
  • Monitor and evaluate the effectiveness of case plans and make adjustments as needed

Requirements

  • Relevant bachelor's degree, or a relevant diploma and be working towards a degree
  • Previous complex case management experience within an Intensive Therapeutic Care or Out Of Home Care setting
  • Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect
  • Understanding of current casework practices for children and young people
  • Understanding of trauma and its effects on children and young people
  • Current drivers' licence
