Care Adviser Role

This is an exceptional opportunity for you to contribute meaningfully as a Care Adviser. You will play a vital role in ensuring seniors receive the services and respect they deserve.

Key Responsibilities:
  • Deliver person-centred care management that respects each client's unique needs
  • Conduct assessments and coordinate home care services, products, and assistive technology
  • Maintain optimal care packages within allocated resources to ensure sustainable service delivery
  • Foster strong relationships with clients, families, healthcare teams, and support services
Requirements:
  • Relevant qualification (Certificate IV in Aged Care, Community Services, or equivalent)
  • Previous experience in Home Care, Community, or Aged Care environments
  • Strong understanding of client-centric service delivery and care coordination
  • Effective relationship-building and problem-solving skills
  • Intermediate computer skills, including MS Office
  • Current driver's licence and willingness to travel within the region
