About the Opportunity

This is a permanent part-time opportunity (0.8 FTE) that allows you to work from home with fast, reliable internet and an appropriate workspace. We'll provide you with the equipment, training, and support you need to succeed in your role and deliver high-quality services underpinned by the latest evidence base.

Your Key Responsibilities

  • Deliver health coaching and an innovative remote cardiac rehab program to clients using a coordinated and integrated approach.
  • Facilitate health behaviour change and improve health outcomes for clients.
  • Work collaboratively with healthcare professionals to ensure seamless client care.
  • Maintain accurate records and meet performance metrics.

About You

To succeed in this role, you'll have:

  • A minimum of 5 years' professional clinical experience as either a Registered Nurse or Physiotherapist with current registration with AHPRA.
  • 2+ years' experience delivering cardiac rehabilitation (phase 2) within the past 5 years.
  • Previous health coaching experience and understanding of health behaviour change methodology.
  • Experience in acute and primary health care and care planning.
  • Solid understanding of cardiac rehab programs, cardiac disease, and evidence-based practice.
