We are seeking a highly skilled and dedicated individual to join our team as a Social Support Program Leader at Latrobe Community Health Service.
About the Role
The successful candidate will be responsible for leading our Social Support program, working collaboratively with the Manager Commonwealth Home Support and Carer Programs to provide a high-quality community and centre-based program of activities.
Key Responsibilities
- Develop and implement a program of activities that maximizes individual choice and aspirations
- Work in collaboration with the Social Support team to provide a high-quality program
- Report to the Manager Commonwealth Home Support and Carer Programs
- Contribute to the development of the Social Support program
Requirements
- Certificate IV or Diploma level qualification in a relevant health field
- Current Working with Children Check
- Current First Aid Certificate Level 2
- Ability to drive the Social Support bus
