About the Opportunity

We are seeking two full-time NDIS Support Coordinators to join our team at CareChoice, a leading provider of disability services. As a Support Coordinator, you will play a critical role in assisting participants with complex needs to build personal capacity and make informed decisions about their NDIS plans.

Key Responsibilities
  • Support participants with complex needs to build personal capacity and make informed decisions about their NDIS plans.
  • Coordinate services in participants' NDIS plans, including managing complex multi-disciplinary teams.
  • Provide advice and consultation to participants, their families, and other stakeholders in response to complex and challenging situations.
  • Develop action plans to resolve barriers arising from complexity and ensure participants are on track to achieve their goals.
About You

We are looking for calm, resilient, and compassionate individuals with a strong understanding of the NDIS and disability sector. You will have a minimum of 1 year's experience in a similar role and possess relevant tertiary qualifications. You will also have excellent interpersonal, negotiation, and complaints resolution skills, as well as a strong understanding of SDA.
