You will be responsible for providing individualized, case-managed care to people living with diabetes and those requiring cardiac rehabilitation. Your role will involve:
- Developing and implementing tailored care plans to meet the unique needs of patients.
- Delivering community and acute support services across multiple locations.
- Participating in outreach programs to support underserved communities.
- Collaborating with multidisciplinary teams to ensure high-quality patient care.
- Acting as a clinical leader to drive innovation and excellence in your specialty area.
As a Diabetes Educator, you will have the opportunity to grow professionally through mentoring, online learning resources, and development opportunities.
To succeed in this role, you will need:
- Postgraduate qualifications in Diabetes Management and/or Chronic Care.
- At least 3 years' experience in a relevant field (consideration will be given to those with less experience).
