Job Title: Disability Support Worker

We are seeking a dedicated and compassionate Disability Support Worker to join our team at National Networks Plus (NNP) on the Sunshine Coast. As a Disability Support Worker, you will play a vital role in supporting individuals with disabilities to live independently and participate fully in their communities.

About the Role:

This is a fantastic opportunity to work with a Not-for-Profit organization that values its employees and provides a supportive and inclusive work environment. As a Disability Support Worker, you will be responsible for:

  • Providing individualized support to people with disabilities to achieve their goals and aspirations
  • Developing and implementing care plans that promote independence and community participation
  • Collaborating with other professionals, including healthcare providers and family members, to ensure comprehensive support
  • Responding to crisis situations and providing immediate support and guidance
  • Maintaining accurate records of interactions, assessments, and program outcomes
Requirements:

To be successful in this role, you will need:

  • Cert 3 in Disability and Aged Care or Cert IV in Youth Care (or working towards)
  • Recent and relevant experience in a similar role
  • 2 professional references
  • NDIS Worker Screening (or willing to obtain)
  • Paid Working with Children check (or willing to obtain)
  • Current First Aid and CPR (or willing to obtain)
  • Unrestricted Australian Drivers License (required if providing community access support, driving facility vehicles)
