Caregiver Opportunities

We are seeking experienced caregivers to join our team in delivering high-quality, person-centered care in a community setting.

About the Role

This is a dynamic position that requires clinical expertise and strong communication skills. You will be responsible for conducting client care assessments, developing and implementing care plans, coordinating care services, and ensuring compliance with relevant legislation and regulations.

Key Responsibilities:
  • Clinical in-home services including care plan assessment, medication management, wound care, chronic disease management, and palliative care.
  • Delivery and supervision of non-clinical team members in a community setting.
  • Intake processing, clinical care planning, and reporting.
  • Conducting client care assessments with a focus on Consumer Directed Care.
  • Coordinate the provision of quality person-centered care according to the client's needs, service/care plans, and available funding.
Qualifications and Requirements

Australian National Police check, Professional Indemnity Insurance, and a valid Driver's License are essential requirements for this role. A Certificate IV in Training and Assessment is highly desirable. Additionally, we require excellent time management and organisational skills, as well as a commitment to ongoing professional development.

A passion for high-quality client-focused care in a community setting, ability to work autonomously or within a team environment, and a willingness to undergo ongoing training and support.
