**Job Description**
This position involves assessing, planning, and implementing care plans to meet the physical, social, and psychological needs of our residents. You will work closely with our multidisciplinary team to ensure seamless delivery of care services. Your ability to communicate effectively with residents, their families, and other healthcare professionals is crucial in this role.
**Responsibilities**
- Assess and develop comprehensive care plans tailored to individual resident needs
- Collaborate with colleagues to implement care plans and provide high-quality care services
- Communicate complex care plan details to incoming residents and their families
- Maintain accurate and up-to-date records of resident care
- Participate in ongoing professional development to enhance skills and knowledge
**Required Skills and Qualifications**
