Job Title: Clinical Care Specialist

About the Role

We are seeking a skilled Clinical Care Specialist to provide high-quality, person-centered care to residents and their families. As a valued member of our team, you will deliver total care, encompassing physical, emotional wellbeing, and safety.

This role offers autonomy to make decisions and work collaboratively with our personal care, lifestyle, and hospitality teams. You will act as a role model by adhering to safe practice measures in medication management, providing leadership, and promoting a harmonious team environment aligned with our values.

Key Responsibilities
  • Deliver person-centered care to residents, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Provide guidance and support to colleagues, promoting a culture of excellence and continuous learning.
  • Participate in ongoing education and training to stay up-to-date with best practices and industry developments.
  • Contribute to quality improvement initiatives, identifying areas for growth and implementing changes to enhance resident outcomes.
Requirements

To be successful in this role, you will need:

  • A Bachelor of Nursing degree and current registration with AHPRA.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Excellent clinical skills, with a focus on patient safety and quality care.
  • A commitment to delivering exceptional customer service and promoting a positive resident experience.
  • The ability to work autonomously and as part of a team, with a strong sense of accountability and responsibility.
