Pathway to Good Health Navigator

This role offers a unique opportunity to contribute to the Albury Wodonga Aboriginal Health Service's brand-new Pathway to Good Health (PTGH) program, ensuring timely and culturally appropriate health assessments for children and young people entering out-of-home care.
The PTGH Navigator will collaborate with health practitioners, VACCA, Dept Communities and Justice, carers, and service providers across the region to deliver respectful and inclusive services.
- The role involves working closely with the PTGH team to identify and address the health needs of vulnerable children and families.
- It requires strong communication and partnership building skills, as well as a willingness to advocate for families and ensure their voices are heard in service delivery.
The position provides an exciting mix of direct service provision and critical planning and administration, empowering children and their families to navigate complex health systems with confidence and trust.
Applicants must be of Aboriginal and/or Torres Strait Islander descent and hold a Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice or be willing to obtain the qualification within a specified timeframe.
A strong understanding of child protection or out-of-home care systems and associated health needs is essential, as is excellent communication and partnership building skills, and a can-do attitude.
The ideal candidate will possess a strong commitment to cultural safety and the ability to work effectively with diverse stakeholders, including health practitioners, community organisations, and government agencies.
