We are seeking a skilled and compassionate Housing Transition Specialist to join our team in providing supportive housing services to individuals experiencing homelessness.

  • Develop and implement personalized transition plans for clients, focusing on empowerment and resilience building.
  • Collaborate with internal teams and external partners to ensure seamless transitions to sustainable housing options.
  • Advocate effectively for clients' needs across housing, health, legal, and community systems.
  • Maintain accurate records and contribute to service development and continuous improvement.
Key Responsibilities:

Transition Planning: Develop strengths-based, solution-focused case plans in partnership with clients to support successful transitions to long-term housing.

Client Advocacy: Advocate across housing, health, legal, and community systems to meet client needs and ensure access to relevant resources.

Collaboration: Collaborate with internal teams and external partners to connect clients with essential services and support.

Record Keeping: Maintain accurate and up-to-date records of client interactions, progress, and outcomes.

Service Development: Contribute to the ongoing development and improvement of housing services, ensuring they remain responsive to client needs.

Requirements:

Qualifications: A degree qualification in Social Work, Psychology, or a related field is required.

Experience: Demonstrated experience in case management within a homelessness or community services setting is essential.

Skills: Strong communication and advocacy skills, along with a collaborative spirit and ability to build positive relationships with clients and stakeholders.

Cultural Competence: A deep understanding of and commitment to inclusive practice, cultural awareness, and social justice.

Current Driver's Licence: Possession of a current Australian Driver's Licence is necessary.

Working with Children's Check: A clear Employee Working with Children's Check is required.
