We are seeking skilled and passionate professionals to deliver trauma-informed, early intervention case work.
This involves providing a mix of long-term and short-term support services, including school and/or home-based case management, needs assessments, practical assistance, referrals, and group-based interventions.
Our ideal candidate will have strong experience working with children and young people, a demonstrated understanding of trauma-informed practice, and family-focused care. They will be confident engaging with diverse communities, including Aboriginal and Torres Strait Islander and CALD families.
- Respond to referrals and conduct initial needs assessments.
- Develop and implement individualized case plans, provide practical support, and facilitate referrals to internal and external services.
- Provide ongoing case management to clients to achieve identified goals.
- Coordinate and deliver group programs.
- Maintain accurate and timely case documentation, including risk assessments, progress reports, and data entry.
- Work collaboratively with internal teams and external agencies to advocate for clients, participate in case reviews, and support community development.
