About the Role

We are seeking highly motivated and experienced professionals to join our team as Senior Case Managers - Clinical and Complex Case Managers - Senior Clinical. These roles are critical to the delivery of high-quality, veteran-centric case management services to our clients.

Key Responsibilities
  • Manage complex client caseloads and provide support and advice to junior case managers as required.
  • Develop and implement case management plans that address the unique needs and goals of each client.
  • Collaborate with internal and external stakeholders to ensure seamless delivery of services and achieve positive outcomes for clients.
  • Contribute to the development and implementation of branch procedures and policies related to case management.
Requirements
  • Bachelor's degree or higher in a relevant health field, including Nursing, Psychology, Occupational Therapy, or Social Work.
  • At least 2 years of post-grad experience in a relevant role for Senior Case Manager - Clinical, and at least 5 years of experience working as a senior case manager or equivalent senior role for Complex Case Manager - Senior Clinical.
  • Registration with the Australian Health Practitioners Registration Agency (AHPRA) or eligibility for registration with the Australian Association of Social Workers (AASW) is highly desirable.
