Job Summary

We are seeking a highly skilled and experienced Healthcare Coordinator to join our team at WA Country Health Service. As a key member of our Sub Acute Care team, you will be responsible for providing leadership, supervision, and administration of our Sub-acute programs.

Key Responsibilities
  • Coordinate, administer, and evaluate Sub-acute care services to ensure high-quality patient care.
  • Provide advanced clinical skills to direct assessment and care of clients within the multi-disciplinary team.
  • Support the implementation of strategies, principles, and practices of the Sub-acute Care Plan.
  • Collaborate with healthcare professionals to develop and implement patient-centered care plans.
  • Monitor and report on program performance, identifying areas for improvement and implementing changes as needed.
Requirements
  • Relevant qualifications and experience in healthcare coordination or a related field.
  • Strong leadership and communication skills, with the ability to work effectively with diverse stakeholders.
  • Excellent clinical skills and knowledge of Sub-acute care principles and practices.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Strong analytical and problem-solving skills, with the ability to identify and implement solutions to complex problems.
