Job Summary
We are seeking an experienced Registered Nurse to join our team of healthcare professionals. As a Registered Nurse with Right At Home, you will have the opportunity to provide holistic care to our clients, working in a variety of settings including aged, disability, transitional, and palliative care.
Responsibilities
- Provide high-quality, patient-centered care to clients in their homes
- Develop and implement individualized care plans to meet the unique needs of each client
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Conduct regular assessments and evaluations to monitor client progress and adjust care plans as needed
- Provide education and support to clients and their families on health-related topics
Requirements
- AHPRA registration as a Registered Nurse
- 3+ years of community nursing experience
- Excellent written and verbal communication skills
- Ability to work autonomously and as part of a team
- High level of time management and organizational ability
