Clinical Care Coordinator Role

Our team seeks a skilled and compassionate Registered Nurse to provide leadership and support in delivering high-quality clinical care to our residents.

  • This includes promoting and guiding advanced, holistic, and evidence-based practices in line with contemporary standards, and ensuring appropriate clinical supervision of staff in collaboration with the Service Manager.

The ideal candidate will have experience working in aged care, strong interpersonal skills, and effective communication abilities. They should be competent in computer literacy, specifically Microsoft Office Suite, and have knowledge of ACFI.

About the Opportunity

This is a full-time position with participation in an on-call roster for after-hours support. Relocation assistance is available if required.

Responsibilities
  • Consulting with residents/relevant representatives, allied health professionals, and other stakeholders;
  • Managing care plans, including wound care and outcomes within the facility;
  • Supervising staff clinical practice in line with best practices and policies;
  • Demonstrating effective communication with all stakeholders through established channels;
  • Promoting teamwork within the care team and across service types;
  • Ensuring all documentation complies with policies, procedures, ACFI, and accreditation standards;
  • Following WH&S guidelines, including infection control and safety reporting mechanisms.
About You

To succeed in this role, you will need to be a Registered Nurse with current registration with AHPRA and demonstrated aged care experience. Strong interpersonal skills and effective communication abilities are essential.
