We are seeking a highly skilled Clinical Case Manager to join our care team at Ldk Seniors' Living. As a key member of our multidisciplinary team, you will play a vital role in supporting integrated and coordinated resident care and service requirements.
Key Responsibilities- Streamline care for your portfolio of residents, ensuring seamless transitions and timely assessments.
- Conduct comprehensive clinical assessments, identifying precise care needs and developing effective care plans.
- Oversee resident support, collaborating with internal and external stakeholders to ensure high-quality care delivery.
- Facilitate smooth transitions to Ldk Homecare AP, where needed, and welcome new residents to the Village.
- Assist residents in accessing externally funded care, navigating Commonwealth Home Care Packages and Support Programs.
- Coordinate aged care assessments through ACAT/MyAgedCare, handling Veterans' care coordination, and assessing eligibility for external care services.
- Collaborate with the Clinical Case Manager Lead to meet resident goals and preferences, ensuring a person-centered approach to care.
- AHPRA registration (Registered Nurse)
- Experience in the community care sector/home care packages, with aged care nursing experience desirable.
- Proven ability to work as a member of a multidisciplinary team, with excellent communication and interpersonal skills.
