Nurse Care Coordinator

As an Enrolled Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team. You will work closely with Registered Nurses and other healthcare professionals to implement and evaluate patient-focused care for patients, families, and carers.
You will be responsible for delivering person-centred care that prioritises the unique needs and preferences of each patient. This may involve working collaboratively with multidisciplinary teams to develop and implement individualised care plans.
The successful candidate will have excellent communication skills, both written and verbal, and the ability to work effectively in a dynamic and fast-paced environment. They will also possess strong organisational and time management skills, enabling them to prioritise tasks and manage multiple responsibilities efficiently.
Key Responsibilities- Delivering high-quality patient care that is tailored to the individual needs of each patient
- Working collaboratively with multidisciplinary teams to develop and implement care plans
- Communicating effectively with patients, families, and carers to ensure their needs are met
- Contributing to the development and implementation of new policies and procedures that enhance patient care
- Current registration to practise as an Enrolled Nurse (Division 2) with the relevant governing body
- Experience working in acute nursing environments
- Strong communication and interpersonal skills
- Ability to work effectively in a dynamic and fast-paced environment
- Strong organisational and time management skills
