Role Description

The role of Clinical Coordinator involves assessing, coordinating and managing patient admissions in a Home Health Initiative. This position operates under a care coordination model, requiring effective communication with in-patient treating units, specialist roles, multidisciplinary teams and community organisations to ensure efficient patient management and discharge planning.

Key responsibilities include promoting the service within the organisation and external stakeholders, supporting efficiency and innovative ways of working with electronic medical records, and demonstrating core nursing values that guide patient care, treatment and management. Autonomous decision making and demonstrated leadership are essential components of this role.

Job Responsibilities

  • Assess and coordinate patient admissions
  • Liaise with in-patient treating units, specialist roles and multidisciplinary teams
  • Promote the service within the organisation and external stakeholders
  • Support efficiency and innovation in electronic medical record use

Qualifications and Experience Required

  • Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
  • Desirable experience in acute or ambulatory/community settings
  • Current Victorian Driver's Licence
