Job Title: Advanced Care Coordinator

We are seeking an experienced Advanced Care Coordinator to join our team and contribute to end-of-life care initiatives. The successful candidate will be responsible for planning, coordinating, and implementing projects related to advance care planning and palliative care.

Key Responsibilities:
