About the Role:
This position sits within the SVHM Healthcare for the Homeless program and will provide an in-reach primary health service to the Elizabeth St Common Ground (ESCG) service run by Launch Housing in partnership with Unison.
Key Responsibilities:
Undertake scoping of acute and chronic health needs of those residing at ESCG
Continue the implementation of the Primary Health model of care, in conjunction with SVHM Healthcare for the Homeless department and Launch housing ESCG team
Work collaboratively with the Launch Housing team to foster a joint understanding of the primary health service at the site
Requirements:
Registered Nurse, with 7 years' experience in acute/community setting
Eligible for registration/membership with AHPRA
Current Victorian Driver's Licence
Advanced clinical skills and experience in an area of mental health, alcohol and other drug use, working with residents with complex psychosocial needs
