Job Description

We are seeking a values-driven Case Manager to provide case management, secondary consultation or co-case management support to participants supported by the HCMS programs.

The ICMI position, sits within the Homeless Case Management Service (HCMS) team. HCMS provides a broad, integrated range of services to the most disadvantaged adults experiencing homelessness in Melbourne's Inner Metropolitan areas.

The Intensive Case Management Initiative (ICMI) Program's focus is on providing intensive case management and outreach support to clients using a relationship-building model to develop for each individual, a safe and secure living environment within the community.

The primary objective of the ICMI Program is to improve a participant's ability to function in the community by addressing mental health care needs and securing and maintaining long-term stable housing and ensuring continuity of service provision with other relevant community service agencies.

Required Skills and Qualifications

You will have:

  • A degree qualification and higher education qualifications in Social Work, Psychology, or a related field
  • Relevant experience in the social service sector including case-managing clients
  • Able to demonstrate knowledge of homelessness including context, causes, models and approaches relating to best practice
  • Experience working with internal and external stakeholders to both maintain and create new connections in the sector
  • A current Australian Driver's License is required
  • Proof of Covid Vaccination is required
  • A National/ International Criminal History Check is required
  • Working with Children check is required
