Key Responsibilities:
- Deliver clinical care in the home environment, supporting clients to stay living in their home and connected to their community.
- Embody a person-centred approach, recognising and promoting the individual's unique goals and preferences.
- Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered.
- Organise and conduct case conferences and maintain open communications with clients and their families, carers and authorised representatives.
- Effectively prioritise client needs in collaboration with other medical and health professionals as required.

About You:
- We are looking for a nurse with a passion for delivering high-quality care and support, excellent communication skills, and the ability to work independently and as part of a team.
