Community Health Nurse

We are seeking an experienced and compassionate Community Health Nurse to join our team at Right at Home Northern Rivers. As a key member of our clinical team, you will be responsible for providing high-quality nursing care to our clients in their homes, promoting their independence and well-being.
Key Responsibilities- Assessment and Planning: Conduct thorough assessments of clients' needs, develop and implement personalized care plans, and ensure that care is delivered in accordance with their goals and preferences.
- Clinical Care: Provide a range of clinical services, including wound management, continence care, catheter care, palliative care, and medication administration, to ensure that clients receive the best possible care.
- Collaboration and Communication: Work closely with clients, their families, and other healthcare professionals to ensure that care is coordinated and effective.
- Administrative Tasks: Maintain accurate records, complete reports, and perform other administrative tasks as required.
- Qualifications: A relevant tertiary qualification in nursing, mandatory compliance, and proven practical experience.
- Registration and Certification: Full, unrestricted AHPRA registration, current Indemnity Insurance, and current First Aid and CPR Certificate.
- Skills and Abilities: Excellent communication, clinical reporting, and clinical problem-solving skills, as well as the ability to work independently and as part of a team.
- Clearance and Certifications: Blue Card and NDIS Workers Screening Clearance, COVID-19 Vaccinations, Influenza Vaccination, National Criminal Check, and Comprehensive Vehicle Insurance.
