Role Overview

We are seeking a skilled Complex Case Manager to play a crucial role in providing support to vulnerable clients navigating Department of Veterans Affairs services.

The Client Support Program is a voluntary time-limited case management program that uses a clinical lens and trauma-informed approach to support clients. You will be responsible for streamlining communication between clients, their families, and the Department, providing a single point of contact for all services within the Department, and facilitating access to external support services.

You will work closely with clinically trained and generalist staff at the APS 6 & EL 1 levels, managing caseloads of varying sizes due to the complexity of clients' presentations and needs.

Main Responsibilities:

  • Act as a primary point of contact for case-managing clients and coordinating access to services through the intake system.
  • Assist clients in accessing required information and support services for their health, mental health, and medical needs.
  • Elevate high-risk and high-profile issues to senior leadership through EL1 Senior Clinicians and/or EL1 Assistant Directors.
  • Liaise with internal stakeholders across all Department business areas to build strong relationships and streamline access to benefits and services for case-coordinated clients identified as being at risk and/or with complex multiple needs.
  • Facilitate service provision to case-coordinated clients by liaising with the Australian Defence Force, ex-service organisations, community groups, and service providers.
  • Link clients to appropriate mental health and other services, such as Open Arms Veterans & Families Counselling.
  • Provide guidance and coaching support to Coordinated Client Support Branch colleagues and other Department staff, as required, to assist in managing challenging client presentations and risk escalations.

Requirements

  • Australian citizenship is mandatory for employment in the Australian Public Service.
