Community Support Specialist

Job Description:

We are seeking a dedicated and compassionate Community Support Specialist to join our team. As a key member of our staff, you will be responsible for providing individualized support and services to our clients in their homes.

Key Responsibilities:

  • Develop and implement personalized care plans that cater to the unique needs and goals of each client.
  • Provide hands-on support with daily living activities, including personal care, meal preparation, and household management.
  • Assist clients with accessing community resources and services, such as medical appointments, transportation, and social activities.
  • Maintain accurate records and reports on client progress and service delivery.

Requirements:

  • Diploma or Bachelor's degree in Community Services or a related field.
  • Minimum 1 year of experience working with vulnerable populations, preferably in a community-based setting.
  • Current and clear Working with Children Check (WWCC).
  • Ability to work collaboratively as part of a multidisciplinary team.
