About the role:

Care Planning and Coordination

A Care Partner plays a vital role in ensuring clients receive high-quality care by overseeing and coordinating complex needs through assessment, planning, and service coordination.

This involves achieving the best care outcomes for each client through activities such as:

  • Developing and reviewing comprehensive care plans in collaboration with the client and multidisciplinary team
  • Managing resources and facilitating care activities
  • Liaising with clinical teams to monitor and respond to changing client needs
  • Continuously monitoring, reviewing, and evaluating care plans and services
  • Assisting clients in navigating the care system and linking them to necessary resources where required

With every day presenting new opportunities, this role requires creativity, flexibility, and a commitment to delivering exceptional care. You will spend most of your time on the road, engaging with clients in their communities.
