Job Role: CHSP Project Officer

The successful candidate will be responsible for coordinating and administering Domestic Assistance and Home Meals services in line with CHSP guidelines.

  • Ensure smooth program operations and high-quality, client-focused service delivery through effective communication with Clients, Contractors, and Meal Vendors.
Key Responsibilities:
  1. Support client intake, monitoring satisfaction, maintaining accurate records, ensuring compliance with funding requirements.
  2. Contribute to community outreach and administrative support to meet each client's goal-directed needs.
Referral Assessment and Client Intake:
  1. Manage client referrals via the My Aged Care Provider Portal and support the intake process for CHSP – DA and Home Meals services.
  2. Conduct assessments and develop Support Plans based on individual client needs, preferences, and funding eligibility.
Service Coordination and Scheduling:
  1. Coordinate the delivery of Domestic Assistance and Home Meals services in accordance with CHSP guidelines and support plans.
  2. Collaborate with Rostering teams, Contractors, and Meal vendors to ensure accurate, timely, and uninterrupted service provision.
Administrative and Invoicing Management:
  1. Review and verify contractor invoices for Domestic Assistance services, addressing discrepancies as needed.
  2. Prepare, process, and maintain invoicing records for Home Meals services.
Communication and Stakeholder Engagement:
  1. Liaise with clients, their families, contractors, and vendors to coordinate services and resolve operational matters.
  2. Respond to phone and email enquiries regarding service delivery, payments, and general program information.
Program Compliance and Quality Assurance:
  1. Ensure services are delivered in accordance with CHSP guidelines, the DA and Meals Client Handbook, and AVWA policies.
  2. Support quality audits and maintain compliance-related documentation.
Community Engagement and Promotion:
  1. Support community outreach and contribute to new client intake efforts.
  2. Assist in planning and delivering marketing activities and annual client satisfaction surveys.
General Support:
  1. Provide backup assistance to colleagues to ensure continuity of service delivery.
  2. Undertake other duties as directed by management to support program success and organisational objectives.
