
Community Services Professional

As a Community and Client Services Coordinator, you will be responsible for delivering high-quality person-centred services to clients while ensuring that service standards are met and programs are operated within program guidelines.
You will coordinate the delivery of services and supports, provide holistic approaches to service coordination, monitoring, and leadership, and report regularly to senior coordinators or service managers.
Key responsibilities include:
- Managing caseloads of clients, remaining within budgetary constraints, and developing support or service plans with tailored strategies and actions.
- Undertaking assessments of client needs, their families, and carers to develop clear understanding of preferences and needs.
- Identifying, coordinating, managing, and/or facilitating range of supports to meet identified needs in accordance with client requirements and program guidelines.
Additional responsibilities include providing personal care, living skills, domestic assistance, community participation, transporting clients, and other services support as applicable. You will also supervise direct reports to ensure timely, individualised, and appropriate support services to clients.
The ideal candidate will have excellent communication and interpersonal skills, be able to maintain confidentiality and uphold client rights, and work collaboratively with others to achieve outcomes.
Required Skills and QualificationsTo be successful in this role, you will need:
- Relevant qualifications in social work, health, education, or related field.
- Proven experience in coordinating services and supports for vulnerable populations.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a team environment and maintain confidentiality.
