Care Partner Job Overview

As a Care Partner, you provide compassionate guidance to customers throughout their care journey. You collaborate with families, clinical teams, and external providers to develop holistic health plans, coordinate services, and ensure safe person-centred care.

This role involves working full-time at our Mulgrave or Clayton branch.

Key Responsibilities:
  • Establish strong relationships with customers and support them achieve their goals at home.
  • Conduct assessments, develop comprehensive health plans, and coordinate multidisciplinary services.
  • Work closely with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
  • Support customers through care transitions, including hospital discharge or changes in health needs.
  • Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
  • Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
  • Promote safety, quality, compliance, and a culture of continuous improvement across all activities.
Required Skills and Qualifications:
  • Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
  • Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
  • Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems.
  • Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning.
  • Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
  • Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers.
  • Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
