Care Services Leader

The Care Services Leader is a crucial role within our team, responsible for empowering individuals to manage their own well-being and optimize their strengths. The successful candidate will be working as a leader in the assessment, planning, provision, and evaluation of high-quality home-based care services.

About You:

  • Develop user-friendly and client-focused practices and care partnerships that encourage individual Home Care consumers to self-determine and maximize their level of independence.
  • Ensure home-based consumers are assessed regularly, with care plans maintained to reflect actual care provided, including input from medical and allied health professionals.
  • Case manage Cultura's Home Care consumers, ensuring individualized care and services, regular reviews, and care changes are affected.
  • Participate in program budgeting to ensure efficient use of resources.
  • Hold at least Certificate IV in Community Services, with a preference for a Diploma, and have recent experience managing/delivering direct care/home-based care services to elderly individuals.

Key Responsibilities:

  • Client Assessment: Conduct thorough assessments to identify consumer needs and develop care plans.
  • Community Liaison: Build strong relationships with community stakeholders to access support services and resources.
  • Problem Solving: Collaborate with consumers, families, and healthcare professionals to resolve complex issues.
