Job Title: Youth Disability Case Manager

Endeavour Foundation is seeking a dedicated and passionate Youth Disability Case Manager to join our team in South Brisbane. As a key member of our team, you will play a vital role in supporting young people with disabilities, aged 12-25, who are engaged with the child protection system and living in 24/7 Supported Independent Living.

Key Responsibilities:
  • Lead a small team of Youth Disability workers, promoting person-centred practices and fostering a culture of inclusivity and respect.
  • Develop and oversee Individualised Support Plans (ISPs) for young people, empowering them to lead their lives and achieve their goals.
  • Identify and manage risks related to each young person's support, ensuring their safety and well-being at all times.
  • Collaborate with key stakeholders, agencies, and NDIS coordinators to align services with goals and ensure seamless support for young people.
  • Provide advocacy across health, education, employment, and community involvement, promoting the rights and interests of young people with disabilities.
  • Monitor service delivery and encourage participation in meaningful activities and pursuits, promoting independence and self-advocacy.
  • Train, mentor, and model person-centred practices for staff, ensuring a culture of excellence and continuous improvement.
  • Maintain accurate records, case notes, and reports, ensuring confidentiality and adhering to data protection policies.
  • Create a safe, supportive, and homely environment, promoting structure and routine, and supporting young people experiencing AOD issues and providing appropriate interventions.
  • Coordinate with disability services to enhance capacity building and daily support, ensuring a seamless transition to adulthood.
Requirements:
  • Bachelor's degree in human services or a related discipline.
  • Minimum 2 years' human services experience in Case Management, with a proven track record of working with young people with disabilities.
  • Previous experience working with young people within a disability context, with demonstrated experience of trauma-informed practice.
  • Excellent interpersonal skills, and the ability to communicate effectively to a diverse range of people.
  • Proficient computer skills and experience using Word, Excel, and Outlook.
  • Hold or be willing to obtain a Working with Children Check (Blue Card), NDIS Worker Screening Check (Yellow Card), Child Safety and Personal History Screening Check (LCS2), First Aid, CPR, National Criminal History Check (Fit2Work), and an open Driver's License with comprehensive insurance on your vehicle.
