Clinical Nurse Consultant

About the Role:

This key position plays a crucial part in the cancer care community, providing collaborative clinical and supportive care for individuals and their families. The Clinical Nurse Consultant will be responsible for developing and implementing comprehensive care management plans, focusing on complex needs and care coordination.

The role involves providing expert consultation and advice on matters related to cancer care across the continuum of care until discharge, with follow-up as needed. Effective communication with medical practitioners, nursing, and allied health professionals is essential, participating in multidisciplinary team meetings to ensure seamless care delivery.

Responsibilities:
  • Develop and implement individualized care management plans, prioritizing complex needs and care coordination.
  • Provide expert guidance and support on cancer-related matters, ensuring continuity of care until discharge and beyond.
  • Maintain open communication with healthcare professionals, participating in team meetings to optimize patient outcomes.
  • Coordinate treatment schedules and triage patient needs, facilitating timely referrals to allied health and support services.
  • Promote data accuracy by contributing to treatment planning and entering relevant information into databases.
  • Foster education and training initiatives, collaborating on research projects related to cancer care services.
Requirements and Qualifications:
  • Adequate vaccination requirements for a Category A role.
  • Current Working With Children Check (WWCC).
  • AHPRA Registration.
  • At least five years of full-time equivalent post-registration experience, with three years in Oncology, Palliative Care, or Cancer Stream nursing; postgraduate qualifications relevant to the field.
