Care Coordinator Role
">We are seeking a dedicated and compassionate Care Coordinator to join our team. As a key member of our care team, you will play a vital role in building strong connections with clients and their families.
">Responsibilities include:
- Conducting thorough client assessments and creating personalized care plans that cater to their unique needs and goals.
- Maintaining accurate records and meeting the necessary standards for health and welfare.
- Providing support and feedback to home care workers to ensure the best possible outcomes for our clients.
- Re-organizing priorities as needed to meet the demands of a fast-paced environment.
The ideal candidate will have experience in assessing and planning care needs, possess sound knowledge of relevant quality standards, and demonstrate excellent organizational skills.

Requirements:
">- Experienced in assessing and planning care needs.
- Sound understanding of aged care quality standards and NDIS practice standards.
- Organizational, administrative, and time management skills.
- Covid 19 vaccinations.
- Registered Nurse or Enrolled Nurse (desired).
Able to obtain:
">- National Police Check.
- Blue Card.
- Driver's License.
- NDIS Worker Screening Check.
- First Aid & CPR Certificate.
Key Responsibilities:
">Client Focus: Build strong relationships with clients and their families through effective communication and empathy.
">Care Planning: Develop and implement personalized care plans that meet client needs and goals.
">Record Keeping: Maintain accurate and up-to-date records of client information and care activities.
