Job Title: Health Care Advocate

This is a fantastic opportunity for passionate health/aged care professionals to take the next step in their career. As a Health Care Advocate, you will play an integral part of our team, providing support and guidance to clients and home care workers.

Key Responsibilities:
  • Build strong relationships with clients and their families to understand their needs and preferences.
  • Conduct comprehensive assessments to develop tailored care plans that support clients to remain in their homes.
  • Provide clinical advice and direction to a team of home care workers to ensure high-quality care.
  • Liaise with clients and their families regularly to address concerns and improve service satisfaction.
  • Maintain accurate and up-to-date client records and information.
Requirements:
  • Formal tertiary qualification in nursing or equivalent.
  • Current AHPRA registration as a Registered Nurse.
  • Demonstrated knowledge of the aged care sector.
  • National criminal history/police check.
  • Working with Children Check.
  • Valid First Aid and CPR certificate.
