
Personal Assistant to Individuals with Disability

Activ’s transition home provides temporary support for individuals with disability, requiring psychosocial support as they transition from hospital to their future permanent residence.
The role is highly rewarding and vacancies are rare.
We have a permanent overnight shift available. Overnight shifts require staying awake overnight working alongside another support worker.
Shifts run from evening until early morning with 7 shifts fortnightly, including one weekend.
Responsibilities of the Role
- Delivering domestic supports in high-needs customer residences;
- Supporting person-centred plans identifying customers’ life goals;
- Ensuring positive behaviour support plans are followed;
- Administration duties, expense, and mandatory reports.
- Be qualified in Cert III in Individual Support (CHC33015 or CHC33021).
- Experience supporting behaviours of concern, bowel care, catheter, diabetes, epilepsy, mental health, and psychosocial support would be beneficial.
- Passion for providing service and assistance, helping others live their best lives;
- Willingness to assist customers with personal care and domestic duties.
- Qualified in Cert III in Individual Support (CHC33015 or CHC33021)
- Experience supporting behaviours of concern, bowel care, catheter, diabetes, epilepsy, mental health, and psychosocial support beneficial
- Passion for providing service and assistance, helping others live their best lives;
- Willingness to assist customers with personal care and domestic duties
