Job Title:
Care Facilitator Role: Support Services Coordinator
We are seeking a compassionate and supportive individual to join our team as a Care Facilitator. The successful applicant will be responsible for providing high-quality support services to clients living at home on the Central Coast.
About the Role:
- Support clients and their families to engage in short-term, specialised care and support services.
- Help clients regain their functional independence at home after a hospital stay.
- Empower clients through individualised care planning and goal setting focused on well-living approach.
- Home visiting clients in the community who have been referred for Transitional Aged Care Program case management and services at home.
- Participate in the development, implementation and evaluation of services and programs that meet clients' individual needs.
Key Responsibilities:
- Provide emotional support and companionship to clients.
- Assist with daily tasks such as grocery shopping and household chores.
- Help clients maintain their independence by supporting them with medication management and personal care.
- Collaborate with healthcare professionals to ensure clients receive comprehensive care.
- Monitor client progress and report any concerns to the relevant authorities.
Requirements:
- Bachelor's Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management), or equivalent knowledge and experience.
- A valid Australian driver's licence.
- Intermediate level of computer skills.
- Excellent time management and organisational skills.
- Proven experience in operating within funding constraints and achieving service delivery objectives within government program guidelines.
