About the Role
  • Conduct home-based assessments, including daily living, mobility, and home safety, to identify areas for improvement and develop personalized care plans.
  • Focus on falls prevention and contribute to the continuous improvement of our program.
  • Develop and implement care plans in consultation with clients and their families to ensure the highest quality of care.
  • Establish and maintain productive working relationships with government departments, hospitals, and other specialist providers.
Requirements
  • Bachelor's degree in occupational therapy or equivalent.
  • Current Occupational Therapist Registration with AHPRA.
  • Previous relevant experience in an Aged or Community care environment.
