About the Role

We are a major provider of tertiary health services, education and research across three campuses in Melbourne and state-wide. Our highly engaged, diverse and collaborative workforce is unified by our purpose, to help people live healthy and fulfilled lives.

Our Emergency Department serves approximately 90,000 patients per year and we admit around 60 patients per day to our 28 bed short stay unit.

The Emergency Care Coordination Team (ECCT) is a transdisciplinary team based in the Emergency Department and comprises Registered Nurses, Physiotherapists, and Occupational Therapists. ECCT aims to provide comprehensive, generic, and discipline-specific assessments and interventions to appropriate patients with an emphasis on facilitating timely discharge.

ECCT works closely with the Independence Program to achieve effective and safe discharges from the Emergency Department.

Key Responsibilities

  • Providing comprehensive assessments and interventions to patients in the Emergency Department;
  • Working closely with the ECCT team to facilitate timely discharge and improve patient outcomes;
  • Developing and implementing individualized care plans that meet the unique needs of each patient;
  • Collaborating with other healthcare professionals to ensure seamless transitions of care.

  • Excellent assessment skills and a strong understanding of emergency, community, acute, and subacute care;
  • Strong communication and collaboration skills, with the ability to work effectively with a multidisciplinary team;
  • Ability to work in a fast-paced environment and prioritize tasks effectively;
  • Strong problem-solving skills and the ability to think critically in complex situations.
